Original 1990s Guyatone FL-3 Flanger.   It's working great but rough cosmetically with a band that holds it together busted as seen in photos.   Still works great.   Sounds amazing like a vintage Flanger pedal a la Eddie Van Halen, Smashing Pumpkins, etc.
